Abstract

Neonatal jaundice is one of the most common conditions encountered in the early neonatal period, arising primarily from the immature hepatic bilirubin‐clearance mechanisms of the newborn and affecting an estimated 50–60% of term and up to 80% of preterm infants. If untreated, excessive unconjugated bilirubin can lead to serious complications including acute bilirubin encephalopathy and kernicterus. Over recent years, significant advances have emerged in the diagnosis, monitoring, and treatment of neonatal hyperbilirubinemia. This review synthesizes recent developments in three key domains: (i) improved point-of-care and smartphone‐based bilirubin assessment technologies facilitating earlier and more accessible screening; (ii) enhanced phototherapy modalities (including high-intensity blue-LED systems, filtered sunlight therapy, and integration of monitoring and control systems) that increase efficacy and safety of treatment; and (iii) emerging adjunctive and preventive strategies (such as haemolysis-risk stratification, non-invasive bilirubin prediction models, and novel therapeutic agents) that aim to reduce the need for exchange transfusion and long-term sequelae. Together, these innovations promise to reduce morbidity and mortality from severe hyperbilirubinemia, particularly in resource-limited settings. Nevertheless, important gaps remain—among them the need for robust long-term outcome data, implementation in low- and middle-income countries, and cost-effectiveness analyses of newer technologies. In conclusion, by translating these advances into routine clinical practice and strengthening neonatal care systems globally, the goal of safer, more effective management of neonatal jaundice is increasingly within reach.
